3/15/2023 EnchantaVaughn Approved Behavior: Upon arrival, client was witnessed interacting safely with her family and preparing for her in-home visit. Mom was happy client was being respectful and expressed joy for her positive behaviors. Mother reports client continues to struggle with being able to complete chores without her asking her several times. Behavior: Client appears to be content and in a stable mood, as observed by positive and calm demeanor. Client was observed following rules and being compliant of the reinforcement provided by mom and QMHP. During the second portion of the session client continued to struggle with being able to express her feelings when she is angry. Intervention: QMHP modeled proper positive behavioral expectations for the client to utilize in the home. QMHP provided praise when the client continued to display positive behaviors throughout the day, remaining on task and was very respectful to siblings, parent, and mental health professional. QMHP challenged client to continue displaying a positive attitude while employing positive reinforcement when client is able to achieve her goals and objectives. QMHP role played with client on ways to respect her mother the first time she is given instructions to prevent her mother from having to communciate with her often about her behaviors within the home setting. Intervention: QMHP engaged the client in a role reversal exercise and required the client to think about her goals for the future. QMHP activity helped client state, demonstrate, or identify how to communicate her feelings. QMHP reviewed effective communication skills with client to improve the way she spoke to family members. QMHP reiterated with mom the importance of maintaining a healthy relationship with her daughter so that she learns to trust other adults. QMHP role played with client appopriate ways to communicate and express her anger in an assertive manner. Response: Client remained soft spoken as she interacted with QMHP, mom and siblings. Client stated she was glad to be in therapy both at home because she’s learning a lot about herself and her family. Client learned to avoid negative and defiant behavior and comply to rules. Client have adapted and is observed modeling appropriate behavior. Client is making progress with expressing her feelings. Client was receptive to the education involving her mental health. Response: Client expressed positive self-talk, modeled positive behavior, and good communication skills during role play activity. Client did not display any outbursts during the role play exercise. Client showed interest in building interpersonal relationships with peers and authority to increase support system. Client has improved with staying in the home, following rules, and utilizing her breaks appropriately without needing to displaying attention seeking behaviors to get her way.
